Athletic Eligibility Requirements
 
Step 1: Sign into Etrieve – https://etcentral.lanecc.edu/#/form
   
   • Use your student L# and Lane email to complete the steps below. 
   • Select Forms - Go to Student-Athletics 
 
Step 2: Complete the Following Forms
 
Returning Sophomore Lane athletes skip steps 2, 3, and 4. Third year Lane athletes must update any forms that expire after 2 years (Physical Exam & Physical Evaluation). Submitting these forms does NOT automatically make you eligible. You must be cleared by our athletic trainer and athletic department before you are eligible to participate.

  •     ATHLETICS 02 NWAC QUESTIONNAIRE – (1st year Freshmen/Transfers only)
  •     ATHLETICS 03 MEASLES VERIFICATION
  •     ATHLETICS 04 PARENT GUARDIAN RELEASE – (If under 18 years old)
  •     ATHLETICS 05 INSURANCE FORM 
  •     ATHLETICS 06 PRE-PARTICIPATION PHYSICAL EVALUATION 
  •     ATHLETICS 07 PHYSICAL EXAM - Must be After July 1 (Can't be done by a chiropractor) 
Errors/Incomplete Forms:
 
   • Returned forms are sent to your inbox within Etrieve. 
   • Check your Lane email frequently for notifications from us. Select “Returned Form” and “History” to read how to correct your form.   
   • Use the attachment button at the bottom to send your insurance.
 
Additional Information: 
 
PHYSICALS- Clearance from a medical doctor is required before participating. The Physical Exam Form MUST BE SIGNED AND DATED BY A PHYSICIAN AFTER JULY 1st.
 
 
Lane's Health Clinic offers sports physicals. To schedule call 541-463-5665. They are located in Building 18 room 101.
If you are scheduled for a Physical Exam at the Health Clinic you must complete and print the Pre-Participation Physical Evaluation and take to your appointment. 
 
TRANSFERS- A Tracer Report and official transcripts from all former institutions is required if you attended other colleges or university after high school.
